Abstract

The diagnosis of tuberculous pleurisy (TP) can be difficult, and a pleural biopsy is frequently necessary. Enlargement of the internal mammary lymph node (IMLN) has been hypothesised to play a potential function and may be used as a method to raise the pre-test probability of TP. By comparing its prevalence and test results to those seen in patients with infectious, non-tuberculous pleurisy (NTIP) and in controls free from respiratory disorders, the current analysis sought to evaluate the function of IMLN involvement in TP in a multi-centric case-control study (CP). Patients with TP and NTIP from all of Italy's pulmonology units were included in the study. Concordance between chest computed tomography (CT scan) and thoracic ultrasound (TUS) measurements, as well as prevalence, accuracy, and predictive values of ipsilateral IMLN involvement between cases and control groups, were evaluated. In comparison to the NTIP and CP groups, TP had a considerably higher rate of ipsilateral IMLN involvement. Age-specific test performance results showed that patients older than years had a strong positive predictive value and older patients had a high negative predictive value. There was only fair agreement between the CT scan and ultrasonography. When determining the pre-test probability of TP, consideration of IMLN involvement is important. A tool that can direct the diagnostic work-up of suspected TP is essential, especially given the rising global frequency of mycobacterial infections.

Introduction

Extrapulmonary tuberculosis' second most prevalent form is tuberculous pleurisy (TP). It typically occurs as an acute disease, especially in young immunocompetent patients, and shows up as a unilateral, exudative effusion. The most typical symptoms include dyspnea, fever, coughing, and pleuritic chest discomfort. Night sweats, weight loss, and malaise are further clinical indicators. The pathophysiology of TP is associated with direct pleural infection and, less frequently, a delayed hypersensitivity reaction to mycobacterial antigens in the pleural space. The interaction of mycobacterial antigens with T cells results in a delayed hypersensitivity reaction, a cytokine cascade, and macrophage activation, which increase capillary permeability and decrease lymphatic outflow, resulting in the buildup of pleural fluid. The immune reconstitution inflammatory syndrome, a sneaky and possibly dangerous complication of opportunistic infections in immunocompromised individuals when an efficient immune response recovers, is a clear example of how the pathogenesis is caused by the activation of the immune system. Although biomarkers like pleura adenosine deaminase (ADA) and polymerase chain reaction (PCR) may be useful in this context, the diagnostic work-up of TP is typically difficult due to the variability in sensitivity from microbiological tests of pleural fluid, including both direct microscopic examination and cultures. The Monteux test, which is frequently unavailable and unreliable, or interferon-gamma release assays, which frequently yield false-negative results in patients, are other complementary diagnostic procedures. Preliminary findings about the effectiveness of the QIAreachTM QuantiFERON-TB assay on active pulmonary tuberculosis patients show promising. Since Mycobacterium tuberculosis can only be directly isolated from pleural samples and caveating granulomas can only be seen through a transthoracic or medical thoracoscopy approach, this method continues to be the gold standard. With a not insignificant number of false negative cases documented in the literature, it is a relatively intrusive method. In this situation, a tool that might influence the pre-test probability of TP might be useful to direct the diagnostic work-up of these individuals. For this reason, a few studies have looked at the ipsilateral internal mammary lymph node (IMLN), suggesting a potential relevance for this particular clinical trait. This extensive multi-center observational case-control study included fourteen pulmonology units throughout Italy. The coordination ethics committee (approval number) and each local ethics committee were informed of the study protocol's compliance with the Declaration of Helsinki's ethical principles and gave their respective seals of approval. Every research facility that took part in this investigation gathered consent forms for the use of de-identified patient information, and whenever practical, patients who participated in the study were also given a special form. In accordance with the recommendations of our Ethical Committee, a waiver of the informed consent form was used in cases of patient death, patients who had already been discharged, and patients who were lost at follow-up. Cases and two distinct control groups were compared. Patients with NTIP and signs of acute inflammation made up the first group (e.g. fever, fatigue, chest pain, elevated C-reactive protein). Subjects in the second group were those who had undergone at least one CT scan as part of their diagnostic workup and had been admitted to emergency rooms for injuries or other non-respiratory medical conditions (CP group), without a history of malignancy or acute or chronic respiratory illnesses.

According to prior experiences, an internal mammary lymph node was deemed abnormal when it had a small diameter. The investigators at each center and their radiology department analyzed all CT scans. For each case and control, particular clinical traits and demographic information were gathered. Age, gender, laterality of the pleural effusion, the presence or absence of ipsilateral and/or contralateral internal mammary lymph nodes, and/or mediastinal lymph nodes with respect to sizes were some of these. If TUS was available, each center was also asked to provide a TU assessment of IMLN, in order to compare TUS and CT scan findings; concordance was defined as the proportion of patients with IMLNs at both TUS and CT scans. The current investigation is the largest multi-centric case-control study that has been conducted to date, and it evaluates the prevalence of ipsilateral IMLN involvement in TP patients in comparison to NTIP patients and controls who are free of respiratory disorders. The prevalence of ipsilateral IMLN, which is defined as the discovery of a lymph node with a short axis in TP patients, was generally found to be significantly greater than that seen in both control groups. Although not pathognomonic, a pathological ipsilateral IMLN significantly raises the pre-test probability of TP, especially in younger subjects (aged equal to or less than years), while its absence almost completely allows the exclusion of any infectious pleurisy (tuberculous and non-tuberculous) in all subjects and significantly lowers the pre-test probability of TP in older subjects (aged equal to or more than years). The difference in TP and NTIP prevalence between young and middle-aged subjects and the elderly may account for the age-related impact on test performance. Epidemiological studies show that TP is more prevalent in younger populations, typically immigrants, while NTIP primarily affects the elderly because older age and certain pre-existing comorbidities are known independent risk factors for non-tubercular empyema, which are more prevalent in the aging population. Additionally, the lymph nodes in the TP group were generally, and frequently exceeding, those in the NTIP group, according to our results on the mean size of IMLN in patients and controls. A tubercular etiology should therefore be suspected more if an IMLN is present and has a short axis greater than. Last but not least, the strong, albeit not perfect, an agreement between CT and TU evaluations is another crucial finding from our study. The decision to use a CT scan to quantify IMLN prevalence rather than a TUS, which is more likely to be influenced by operator expertise and is not available in every center, was made due to the higher diffusion and reduced measurement variability of this technology. TUS, however, has the advantage of being non-ionizing, quicker, and easily accessible at the patient's bedside and in an outpatient setting; as a result, in the presence of adequate skills, it may be the first approach to assess IMLN involvement, saving patients with suspected pathological lymph nodes from receiving a more accurate CT scan evaluation with relative measurements. Although IMLN involvement has been looked into in a few prior studies using CT or TUS, none have compared its prevalence to control groups or evaluated its predictive usefulness in the diagnostic work-up of TP. As was previously mentioned, it is difficult for doctors to diagnose TP because clinical and radiological features are frequently nonspecific, pleural fluid microbiological analyses have a high rate of false negative results, and the acquisition of tissue—which is necessary for identifying Mycobacterium tuberculosis—is frequently postponed because of the higher risks and the limited availability of technical tools and expertise. These factors have an impact on the prognosis of the patient. Since it significantly raises the pre-test probability of the disease, particularly in younger subjects, the presence of IMLN involvement in this context, even though it is not diagnostic in and of itself, may be very helpful in guiding subsequent diagnostic steps in patients with suspected TP. This suggests the need for an immediate biotic approach. In contrast, the absence of IMLN involvement in elderly patients may enable clinicians to forego invasive therapies with unfavorable risk-benefit ratios. A large number of cases, comparison with two separate control groups, and representativeness of the research population due to the involvement of many locations throughout Italy are major strengths of the current study. But there are some restrictions that need to be understood. Due to challenges in recruiting participants among the participating sites, the study design did not match the age and sex of the cases and controls. However, later analyses conducted using different age subgroups allowed us to reduce this methodological limitation.

Additionally, TU's evaluations were not conducted at every center, and when they were, the results could have been impacted by the operator’s experiences. The present study, although not pathognomonic, first provides evidence that evaluation of IMLN involvement (by CT scan and perhaps TUS) is a critical component to direct diagnostic work-up of patients with suspected tubercular pleural effusion. The pre-test chance of mycobacterial origin is significantly raised when a pathological IMLN is present, especially in young people and when lymph nodes are larger than normal. This strengthens the rationale for more invasive diagnostic techniques. Instead, its absence (defined as an IMLN short axis less than, especially in patients over years of age) almost always indicates the presence of a TP. Given the rising frequency of tuberculosis worldwide, a tool that can direct diagnostic procedures in this difficult situation is crucial, especially for patients who have a poorer risk-benefit profile for a more invasive approach and in areas with few local resources.
